Wellness Coach (Male Wellness Agency)
Job description
Wellness Coach is a supportive partner within our diabetes management program that assists members of the community with work/life balance and other parts of their lives that affect their wellness. The wellness coach aids participants in using a Continuous Glucose Monitor (CGM), building a positive mindset, diabetes self-management, and stress management, as well as in creating and achieving long-term health objectives, and provides guidance as needed during their journey toward well-being.
Roles and responsibilities
· Analyze CGM data to identify participants who are at high risk for poor outcomes.
· Meet with eligible participants to enroll them into the program.
· Assist with all pilot programming and execution.
· Act as health navigator for the participants and be responsible for keeping the participants engaged with the U Got This program.
· Provide direction and communication to participants who need technical support with their CGMs.
· Collect required data from participants and input data into QuickBase on a weekly basis.
· Document all community engagement, referrals, and other outreach efforts with participants in QuickBase
· Understanding and interpreting the data so that participants are guided to the right resources.
· Other duties as reasonably assigned
Qualifications and Education Requirements
· Bachelor’s Degree preferred
· Experience in home health care preferred
· Excellent listening and communication skills.
· Passion for the proper care and well-being of community members.
Job Type: Full-time
Salary: $44,000.00 - $46,000.00 per year
