Program Coordinator
The Program Coordinator is responsible for leading the program’s Wellness Coaches within our diabetes management program that assists members of the community with work/life balance and other parts of their lives that affect their wellness. The Program Coordinator and Wellness Coaches aids participants in using a Continuous Glucose Monitor (CGM), building a positive mindset, diabetes self-management and stress management, as well as in creating and achieving long-term health objectives, and provides guidance as needed during their journey toward well-being.
Roles and responsibilities
Lead a team of wellness coaches who will work with hundreds of participants.
Monitor caseloads of all wellness coaches
Organize and implement wellness coach training.
Facilitate team and partner meetings and support groups for participants.
Coordinate and lead all pilot programming and execution.
Coordinate participant support with community partners and wellness coaches.
Coordinate activities related to logistics, program curriculum, reporting and analyzing program data.
Meet weekly with wellness coaches and leadership to review caseloads and program coordination with partners.
Oversee the collection of the required data from participants and input data into QuickBase on a weekly basis.
Document all community engagement, referrals, and other outreach efforts with participants in QuickBase.
Other duties as reasonably assigned.
Qualifications and Education Requirements
Bachelor’s Degree preferred.
Excellent organizational skills preferred.
Experience with case management
Experience in community nursing or home health preferred.
Comfortable analyzing reports and using data to drive performance results.
