Health Advocate

The purpose of a health advocate is to serve as a supportive and knowledgeable resource for individuals seeking assistance in navigating the complex healthcare system and making informed decisions about their health and well-being. Health advocates work to empower and guide individuals in accessing appropriate healthcare services, understanding their medical conditions, and advocating for their rights within the healthcare system. 

Roles and responsibilities     

  • Work closely at the designated CBOs to recruit eligible participants for the study.   

  • Screen potential participants to determine their eligibility for the study based on the inclusion and exclusion criteria and obtain informed consent.   

  • Train participants in home blood pressure monitoring, ensuring they understand how to use the blood pressure cuffs correctly, support participants sync their blood pressure cuffs with Stel, the digital platform for monitoring.  

  • Assist participants in completing the baseline online survey, ensuring all required information is accurately provided and address any questions or concerns participants may have during the survey completion process.   

  • Provide referrals to primary care facilities for participants who require further evaluation or treatment, for Intervention 1 and 2 coordinate with clinical research team & schedule appointments for participants.   

  • Conduct follow-up study visits at the CBOs at 6, 12, and 18 months to capture blood pressure outcomes and ensure the completion of surveys.  

​​Qualifications and Education Requirements​  

  • Bachelor’s Degree preferred  

  • Excellent organizational skills preferred  

  • Experience with case management  

  • Experience in community nursing or home health preferred  

  • Comfortable analyzing reports and using data to drive performance results
